Important notes

• This submission form is intended for internal whistleblowing reports concerning the NTT Group and its employees. Reports must concern misconduct or compliance issues involving the NTT Group or its employees.
• The Corporate Ethics Committee or the Audit and Supervisory Committee of NTT, Inc. will review the submission to determine whether it concerns misconduct or compliance issues, and will conduct an investigation as necessary.
  If the investigation finds evidence of misconduct, necessary measures will be taken.
• Reports submitted through the online form will first be received by the designated external law firm. Personal information, including names and contact details, will be anonymized before the report is directed to the Corporate Ethics Committee or the Audit and Supervisory Committee of NTT, Inc.
• Information submitted through this form will be protected in accordance with internal policies and used solely for review and investigation purposes.

• You may also choose to submit a report exclusively to the Audit and Supervisory Committee of NTT, Inc. Eligible reports are limited to misconduct or compliance issues involving directors, executive officers, counsellors, special advisors, or members of the Corporate Ethics Committee of the NTT Group.
